Sunrise Home Health Services is seeking a compassionate Home Health RN Case Manager to oversee a Northwest Phoenix Valley caseload, including Sun City, Surprise and Peoria, with a focus on quality patient care in home settings.

You will perform SOC/ROC/Recertifications, coordinate with therapists and physicians, and provide education to patients and families. Strong clinical assessment, communication, and organizational skills are essential.

Qualifikationen

• Current Arizona RN license or Compact license.
• Graduate of an accredited nursing program.
• Current CPR Certification.
• Valid driver's license and reliable transportation.
• Ability to work independently in a home-based setting.

Aufgaben

• Manage an assigned caseload within the Northwest Phoenix Valley.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop individualized plans of care based on nursing assessments.
• Provide skilled nursing care and wound care as needed.
• Manage medications and provide patient education.
• Monitor patient condition and coordinate with physicians and care team.
• Educate patients and caregivers on disease management and safety.
