Visiting Nurse Health System (VNHS) in Decatur, GA is seeking a Registered Nurse Case Manager for Home Health. The role coordinates care for a caseload, ensuring quality standards and utilization guidelines, with a focus on patient education and family engagement.

The RN Case Manager assesses, plans, implements, and evaluates care plans to optimize outcomes in a home-based setting. Requires GA RN licensure and CPR, with strong communication and teamwork skills.

Qualifications

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current licensure by the Georgia State Board of Nursing and CPR certification.
• Possess a valid driver’s license and reliable transportation and current auto insurance.
• Prior home health experience preferred for PRN roles.
• Knowledge of Medicare/Medicaid and funding source regulations for home health care.

Responsibilities

• Develop and implement individualized plans of care with physician orders and interdisciplinary input.
• Modify and update care plans as patient conditions change and evaluate effectiveness.
• Educate patients and families on diseases, self-management, and prevention of hospitalizations.
• Complete and submit OASIS documents, visit notes, and other required records within VNHS timelines.
• Communicate patient status changes clearly to patients, physicians, and team members.
• Offer emotional support and connect patients/families with MSW and community resources as needed.
• Supervise LPNs and HHAs per VNHS policy and ensure care plans meet patient needs.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality home health care in alignment with VNHS policies and nursing standards.
• Maintain competencies through in-services and continuing education.
• Schedule visits to optimize productivity and manage caseload.
• Precept newly hired nursing staff and coordinate with office staff on patient status.
