Job summary

Advanced Home Health & Hospice of St. George seeks a Home Health RN Case Manager to coordinate and manage patient care in a home health setting, delivering skilled nursing services and guiding care plans.

The role requires strong clinical knowledge and excellent communication, with autonomy and collaboration within an interdisciplinary team. The position is PRN/Part-Time, primarily weekdays with occasional weekends, offering flexible scheduling and a supportive team environment.

Qualifications

• Current RN or LPN license in the state of practice.
• Some home health or related healthcare experience preferred.
• Current CPR certification.

Responsibilities

• Conduct comprehensive patient assessments to identify health care needs and update care plans.
• Provide skilled nursing care including wound care, meds administration, and patient education.
• Coordinate and supervise home health aides and other professionals involved in patient care.
• Communicate with patients, families, physicians, and team members about progress and treatments.
• Ensure compliance with regulatory guidelines, documentation, and agency policies.
• Evaluate patient outcomes and adjust care plans as needed.
• Participate in quality improvement and continuing education programs.
