Job summary

Dartmouth Health Home Care is seeking an RN Case Manager to join our team delivering skilled home health and hospice services in the Upper Valley area. The role involves care planning, physician and family collaboration, and ongoing patient assessment.

Shifts are days with on-call requirements, and a $10,000 new hire retention bonus applies. The RN will coordinate care with an interdisciplinary team, supervise LPNs and LNAs, and participate in community-based care.

Qualifications

• Must hold an active RN license in VT or NH, or a compact license.
• Graduate of an accredited school of nursing.
• One year of home health RN experience preferred.

Responsibilities

• Screen, evaluate, and treat patients referred by a licensed provider.
• Develop and review care plans with physicians, patients, and families.
• Coordinate interdisciplinary care and notify discharges in a timely manner.
• Provide ongoing assessment of clinical, functional, and emotional status.
• Supervise LPNs and LNAs in the home.
• Document evaluations and care goals; educate patient and caregiver.
