Job summary

Home Health & Hospice Care is seeking an RN Home Care Case Manager for the Greater Manchester, NH area. You will perform assessments, create care plans, and coordinate with physicians and families to ensure high-quality home-based care.

You will guide patients through care, educate families, and maintain thorough documentation while traveling within your territory. A BN/BSN is preferred, with NH RN licensure and BLS/CPR certification expected.

Qualifications

• RN with an Associate Degree in Nursing and NH licensure.
• At least 1 year of acute care and home health experience preferred.
• Familiar with OASIS, reimbursement, and EMR systems.

Responsibilities

• Perform initial and ongoing home assessments and OASIS.
• Develop individualized care plans with measurable goals.
• Coordinate interdisciplinary care and communicate changes to physicians.
• Educate patients and families on self-care and safety.
• Maintain accurate clinical notes and participate in quality initiatives.
• Travel within assigned territory to support home visits.
