Dartmouth Health Home Care in Vermont is seeking an RN Case Manager to provide home health services across the Upper Valley region. You will screen, evaluate, and coordinate patient care with an interdisciplinary team, developing and updating care plans in partnership with the patient, family, and physicians.

The role offers full-time, part-time, and per diem options with day shifts (8am–4:30pm) and on-call requirements. A $10,000 new hire retention bonus applies (conditions).

Qualifications

• Must be a licensed RN in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing.
• One year of experience as an RN in home health care is preferred.

Responsabilités

Licensed Registered Nurse (VT/NH) or compact license

Graduate of an accredited school of nursing

Responsibilities

The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

• Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
• Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
• Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and DHHC colleagues.
• Document evaluations, treatment goals, and objectives.
• Teach the patient and caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the provider and utilizes opportunities for health counseling with patients and caregivers.
• Provides clinical oversight of LPNs and LNAs in the home.

Qualifications

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing
• One year of experience as an RN in a home health care setting is preferred

Required Licensure/Certifications

• Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.
• Valid driver’s license with a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire
