Job summary

Community Health Network, Inc. seeks a Home Health RN Case Manager to oversee patient care in the community across NH and MA. The role covers admission through discharge, coordinating with families and a care team.

The ideal candidate has at least 2 years in home health case management, performs OASIS visits, and holds active RN licenses in Massachusetts and New Hampshire. Working hours are Monday to Friday with 40 hours per week in a contract capacity.

Qualifications

• Minimum 2 years of recent Home Health RN Case Management experience.
• Experience performing OASIS visits with a Medicare certified agency.
• Must hold both MA and NH RN licenses.

Responsibilities

• Manage and care for a caseload of patients and families in the community from admission to discharge.
• Supervise LPNs and Home Health Aides.
