Dartmouth Health Home Care in Vermont is seeking an RN Case Manager to provide home health services across the Upper Valley region. You will screen, evaluate, and coordinate patient care with an interdisciplinary team, developing and updating care plans in partnership with the patient, family, and physicians.

The role offers full-time, part-time, and per diem options with day shifts (8am–4:30pm) and on-call requirements. A $10,000 new hire retention bonus applies (conditions).

Qualifications

• Must be a licensed RN in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing.
• One year of experience as an RN in home health care is preferred.

Responsabilités

• Screens, evaluates, re-evaluates and treats patients referred by a licensed provider.
• Coordinates and communicates treatment activities with health care team and patient.
• Develops care plans in collaboration with physicians, patient and family.
• Reviews and recertifies patient care plans as appropriate.
• Coordinates care plans through interdisciplinary communication and case conferences.
• Provides ongoing assessment of clinical, functional and emotional status; communicates changes to physician and colleagues.
• Documents evaluations and treatment goals.
• Educates patient and caregiver on self-care and health counseling.

Formation

Licensed Registered Nurse (VT/NH) or compact license

Graduate of an accredited school of nursing

One year RN home health experience preferred
