Specialized Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our team and support patients at home in the Kansas City area. You will develop and oversee each patient’s Plan of Care, coordinate with families and physicians, and ensure timely documentation while promoting independence and satisfaction.

The role emphasizes collaborative care, on-call participation, and adherence to high-quality standards in a warm, supportive environment.

Qualifications

• Minimum 2 years of nursing experience in acute or post-acute care.
• Active State RN licensure required.
• Proof of CPR certification and valid driver's license.

Responsabilités

• Manage patient care from admission through discharge under the Plan of Care.
• Coordinate with patients, families, physicians, and care team members.
• Submit accurate, timely clinical documentation and progress notes.
• Participate in case conferences to review services, documentation, and goals.
• Participate in on-call rotation as assigned.
