Job summary

CenterWell Home Health is seeking a compassionate Home Health Registered Nurse. Reporting to the Clinical Manager, you will provide skilled, one-on-one nursing care in patients' homes, administering medications, performing wound care, and assessing progress to support independence and recovery.

You will develop individualized care plans with physicians, coordinate with a multi-disciplinary team, educate patients and families, supervise Home Health Aides, and maintain accurate documentation.

Qualifications

• Must have current RN licensure and clinical experience (Med-Surg/ICU/ER/acute).
• CPR certification is required.
• Valid driver's license and auto insurance with reliable transportation.
• Ability to work onsite in patients' homes and collaborate with a care team.

Responsibilities

• Provide skilled nursing care to patients in their homes, including meds administration and wound care.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes to the physician and Clinical Manager.
• Educate patients and families on disease management, medications and treatments.
• Maintain accurate records and coordinate with other healthcare professionals.
• Oversee Home Health Aides and ensure documentation is completed correctly.
