Job summary

Specialized Home Care is hiring a Registered Nurse (RN) Case Manager to work with patients in their home environment, leading care planning and following physician orders to improve independence and satisfaction.

You will collaborate with families and clinicians to ensure comprehensive care, document progress, and participate in case conferences. A valid RN license, CPR, and a driver’s license are required, with 2+ years of nursing preferred.

Qualifications

• Minimum 2 years of nursing experience, with preference for 2+ years in acute or post-acute care.
• Active State Registered Nurse licensure required.
• Proof of current CPR certification and valid Driver's license.

Responsibilities

• Manage patient care from admission to discharge and execute the Plan of Care.
• Collaborate with patients, families, ordering physicians, and care team members.
• Submit accurate, timely documentation including progress and clinical notes.
• Participate in case conferences reviewing services, documentation, and goals.
• Take part in on-call rotation as assigned.
