TCI Technology Consulting Inc. is seeking a Registered Nurse (RN) Case Manager for an on-site home health role in Williamsburg, VA.

The position is contract-to-hire and requires coordinating patient care, assessing needs, and developing individualized care plans with a focus on high-quality, patient-centered outcomes. Key duties include comprehensive assessments, collaboration with providers, transition coordination, and documentation of outcomes.

Qualifications

• Active, unrestricted RN license (Virginia or Compact).
• Case Management Certification required.
• Hospice experience is mandatory; home care home base experience a plus.
• Associates or Bachelors in Nursing.
• Clinical nursing and case management experience preferred.
• Strong assessment, coordination, and communication skills.
• Proficiency with electronic medical records and standard computer apps.
• Home Health care delivered in Williamsburg, Yorktown, Gloucester, Hampton Roads, Poquoson, and Charles City counties.
• 8am–5pm, Mon–Fri; on-call weekends; mileage reimbursement provided.

Responsibilities

• Perform comprehensive assessments of high-risk members.
• Coordinate with primary care providers to implement individualized care plans.
• Identify and reassess appropriate, cost-efficient levels of care.
• Arrange non-medical supports to ensure treatment plan adherence.
• Coordinate care transitions and communicate with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources to achieve optimal outcomes.
• Maintain detailed records of clinical, functional, and fiscal results.
• Identify health promotion and illness prevention opportunities.
• Advocate for patients while safeguarding privacy.
