Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager to join our field-based team serving Tidewater Virginia, Newport News, Hampton and Williamsburg. This role blends in-person visits with telephonic outreach to coordinate patient-centered care.

You will conduct assessments, develop care plans, coordinate across providers, review claims history and support complex cases while adhering to regulatory guidelines.

Qualifications

• Active, unrestricted RN license in Virginia.
• Reside in Tidewater, Newport News, Hampton or Williamsburg, VA.
• 3+ years of clinical nursing experience (medical-surgical, acute care, or behavioral health).
• Willing to travel up to 75% of the time; mileage reimbursed.

Responsibilities

• Conduct comprehensive assessments (telephonic and face-to-face) to evaluate medical, behavioral, and social needs.
• Develop and implement personalized care plans to promote short- and long-term wellness.
• Coordinate care across providers, programs, and services for a seamless member experience.
• Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention.
• Support members with complex or multiple conditions using a holistic approach.
• Facilitate referrals to clinical and community resources.
• Review claims history and benefit eligibility to inform care planning.
• Engage members to build trust and encourage active participation in care.
