Job summary

CVS Health is seeking a compassionate RN Case Manager for field-based support across the Tidewater area (Newport News, Hampton, Williamsburg). The role combines in-person visits with telephonic outreach to coordinate patient-centered care, with a 40-hour workweek and daytime schedule.

Requirements include an active Virginia RN license, 3+ years nursing experience, and willingness to travel up to 75% with mileage reimbursement. A BSN is preferred.

Qualifications

• Active, unrestricted RN license (Virginia).
• Residence in Tidewater, Newport News, Hampton, Williamsburg VA areas.
• Willingness to travel up to 75% with mileage reimbursed.
• 3+ years of clinical nursing experience (medical-surgical, acute care, or behavioral health).
• Strong communication, organization, and critical-thinking skills.

Responsibilities

• Conduct comprehensive assessments (telephonic and face-to-face).
• Develop and implement personalized care plans.
• Coordinate care across providers and services.
• Identify risks and gaps in care using data insights.
• Support members with complex conditions via holistic approach.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences with multidisciplinary teams.
• Review claims history and benefit eligibility for planning.
• Engage members to promote active participation in care.
