Job summary

Your Health Organization is seeking a Registered Nurse to serve patients across the Bowman and surrounding area as a Nurse Case Manager. The role involves home visits, coordination with providers, and initiating telemedicine visits to ensure comprehensive patient care.

Candidates should have a current RN license, at least 3 years of clinical experience, and strong communication skills. Travel is required daily, and the position offers a full-time, 8-hour shift schedule (8am-5pm) with competitive

Qualifications

• Must be a licensed nurse in good standing.
• Minimum of three years of clinical experience.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary teams.
• Strong written and verbal communication skills; basic computer knowledge.
• Willingness to travel daily and drive a registered vehicle.

Responsibilities

• Facilitate receiving medical records from patient’s primary provider and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management if eligible.
• Initiate and update a Care Management Plan of Care.
• Coordinate care with providers, facilities, and care teams.
• Document Care Management activities accurately; on-call duties may be required.
