Job summary

Advocate Health is seeking a Registered Nurse for Home Health in the Chicago area. You will build therapeutic nurse-patient relationships, deliver holistic care, and coordinate with families throughout the care continuum.

The role requires participation in on-call rotations, weekend coverage every third weekend, and on-call shifts 3–4 weeknights per month, plus a winter and summer holiday rotation. A sign-on bonus of up to $3,000 is available.

Qualifications

• Graduate from an accredited school of nursing.
• Earn 10 continuing education credits annually.
• One year of medical surgical experience.
• Demonstrated proficiency for the RN role.
• Current Illinois RN license.

Responsibilities

• Develops and maintains a therapeutic nurse-patient relationship across the healthcare continuum.
• Facilitates quality, cost-effective care and mobilizes resources as needed.
• Educates patients and families; supports department education and learning.
• Applies the nursing process to assess, plan, implement, and evaluate care.
• Coordinates referrals and discharge planning with the interdisciplinary team; ensures safety and quality.
