Zusammenfassung

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, and support discharge planning. Under the supervision of an RN or MD, you will help ensure patients receive appropriate care and provide education, coaching, and treatment-decision support, collaborating with a Care Advocate and Social Worker.

Qualifikationen

• Current RN license valid in assigned state(s).
• Current CPR certification.
• At least 1 year of RN experience.
• Valid driver's license and reliable transportation.

Aufgaben

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Provides education and coaching to patients and families.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call duties as assigned.
• Participates in clinical development and continuing education.
