Job summary

Angels Care Home Health in Bristol, VA is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will perform admission visits, determine eligibility for home services, and develop the plan of care in collaboration with the patient, family, and medical team.

You will work with other disciplines, provide professional nursing care, and ensure high-quality outcomes.

Qualifications

• Graduate of accredited diploma, ADN, or BSN.
• Current RN license.
• Current driver’s license.
• One year nursing experience; two preferred.
• CPR certification and hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional nursing care.
• Perform the admission visit to the patient at home and determine eligibility for home care services.
• Develop the plan of care to be followed in collaboration with patient and family.
• Ensure high-quality outcomes through ongoing assessment and care coordination.
