Elara Caring in Boston, MA seeks a Home Health RN Case Manager to join our elite team. You will deliver skilled nursing visits, develop and update care plans, manage an assigned caseload, and coordinate with physicians to ensure seamless patient care at home.

Qualifications

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills; ability to work independently and travel throughout Boston Proper, including Chinatown.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds.

Responsibilities

What You’ll Do

You’ll guide patients through their home health care, from assessment to discharge.
