Job summary

Health Link Home Health & Hospice in the South Bay, CA is seeking a compassionate Registered Nurse (RN) to join our autonomy-focused Case Manager team.

You will perform comprehensive nursing assessments (OASIS), administer meds and IV therapies, coordinate with physicians and the interdisciplinary team, and educate patients and families for safe, effective care at home. A CA RN license and valid driver's license are required.

Qualifications

• Current, active California RN license required.
• Minimum 1 year nursing experience; home health preferred or willing to train hospital/SNF nurses.
• Valid California driver's license and reliable transportation required.
• Strong clinical judgment, organization and ability to work independently.

Responsibilities

• Perform comprehensive nursing assessments (OASIS) including Start of Care, Recertifications and Discharges.
• Administer medications, wound care and IV therapies as part of skilled home health care.
• Coordinate with physicians and interdisciplinary team to ensure the care plan is effective.
• Educate patients and families to manage conditions safely at home.
• Document timely clinical notes on company-issued iPads.
