Job summary

Health Link Home Health & Hospice in San Francisco is seeking a compassionate Home Health Registered Nurse (RN) to join our Bay Area team. This role offers autonomy and field support from clinical leadership and intake teams to guide orders and coordination.

Qualifications

• Current, active California RN License is REQUIRED to apply.
• Minimum 1 year of professional nursing experience; home health preferred.
• Valid California Driver’s License with reliable transportation.

Responsibilities

• Manage caseload with comprehensive nursing assessments (OASIS), Start of Care, Recertifications, Discharges.
• Administer medications, provide wound care (including Vacs), and manage IV therapies.
• Coordinate with physicians and the interdisciplinary team to ensure the care plan is effective.
• Educate patients and families to safely manage conditions at home.
• Document timely clinical notes on company-issued iPads.
