Health Link Home Health & Hospice in the South Bay, CA is seeking a compassionate Registered Nurse (RN) to join our on-site team as a Home Health Case Manager. This role emphasizes autonomy with structured support, performing comprehensive OASIS assessments, care planning, and in-home nursing visits.
You will coordinate with physicians and the interdisciplinary team, administer medications, wound care, IV therapy, and educate patients and families to manage conditions at home.

$135,000 – $156,000 per year

Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
Empower patients and families to manage their conditions safely at home.
Maintain accurate, timely clinical notes on company-issued iPads.
Current, active California RN License is REQUIRED to apply. Applications without a valid CA license number will not be reviewed.
Minimum 1 year of professional nursing experience required. Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.
Valid California Driver’s License and reliable transportation are mandatory.
Strong clinical judgment, organization, and the ability to work independently.
