Health Link Home Health & Hospice in the South Bay, CA is seeking a compassionate Registered Nurse (RN) to join our on-site team as a Home Health Case Manager. This role emphasizes autonomy with structured support, performing comprehensive OASIS assessments, care planning, and in-home nursing visits.

You will coordinate with physicians and the interdisciplinary team, administer medications, wound care, IV therapy, and educate patients and families to manage conditions at home.

Qualificações

• California RN license required
• Minimum 1 year of nursing experience required
• Home Health experience preferred; able to train hospital/SNF nurses

Responsabilidades

• Perform comprehensive nursing assessments (OASIS), Start of Care, Recertifications, Discharges
• Administer medications, wound care (including wound vacs), IV therapies
• Coordinate care with physicians and interdisciplinary team to ensure care plan effectiveness
• Educate patients and families to manage conditions at home
• Maintain accurate clinical notes on company-provided iPads
