Job summary

Health Link Home Health & Hospice seeks a compassionate Home Health RN to join our San Francisco team as Case Manager. You will perform comprehensive nursing assessments and coordinate care with physicians and the interdisciplinary team to support patients at home.

You will administer medications, provide wound care (including Wound Vacs), and document with company-issued iPads. A CA RN license and 1 year of experience are required, with training available for hospital/SNF nurses.

Qualifications

• Current, active California RN License is REQUIRED.
• Minimum 1 year of professional nursing experience; Home Health preferred, but training available.
• Valid California Driver’s License and reliable transportation are mandatory.
• Strong clinical judgment, organization, and ability to work independently.

Responsibilities

• Manage caseload with comprehensive nursing assessments (OASIS), Start of Care, Recertifications, and Discharges.
• Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
• Coordinate with physicians and interdisciplinary team to ensure the care plan is effective.
• Educate patients and families to manage conditions at home.
• Document accurate notes on company-issued iPads.
