Job summary

Health Link Home Health & Hospice in San Francisco, CA is seeking an autonomous, compassionate Registered Nurse (RN) to join our team as a Case Manager.

You will handle comprehensive nursing assessments (OASIS), administer meds, manage IV therapies, wound care, and coordinate with physicians and the care team to ensure the plan meets patient needs. Travel and CA RN license required; we offer supportive leadership and stable compensation.

Qualifications

• Current California RN license is REQUIRED to apply.
• Minimum 1 year of nursing experience; Home Health preferred.
• Valid CA driver’s license and reliable transportation.
• Strong clinical judgment and organizational skills.
• Ability to work independently in patient homes.

Responsibilities

• Manage caseload with comprehensive nursing assessments (OASIS).
• Administer medications and provide wound care (including wound vacs).
• Coordinate with physicians and the interdisciplinary team to ensure the care plan works.
• Educate patients and families to manage conditions safely at home.
• Document timely clinical notes on company-issued iPads.
• Assist with IV therapies as part of home health care.
