Your Health Organization seeks a Registered Nurse to serve patients across the Savannah area as a Visiting Nurse (Nurse Case Manager). The nurse will visit patients and coordinate with providers, traveling to homes and facilities to evaluate patients and initiate telemedicine visits.

This full-time role supports primary care teams, requires a valid RN license, and offers competitive compensation, benefits, and opportunities for professional growth.

Qualifications

• Must be a licensed nurse with RN preferred and license in good standing.
• 3+ years clinical experience preferred.
• Experience in community settings preferred.
• Strong written and verbal communication skills.

Responsibilities

• Facilitate receiving medical records from patient primary providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management and initiate Plan of Care.
• Coordinate and communicate care with providers and families.
• Travel daily to patient homes and facilities for visits and telemedicine.
• Document Care Management activities accurately and up to date.
• On-call may be required.
