Job summary

Your Health is seeking a Registered Nurse to serve wound care patients in the Brunswick area. The Visiting Nurse (Nurse Case Manager) travels to patients’ homes and facilities to evaluate and coordinate care with providers.

This full-time role includes 10-hour shifts Monday-Thursday and daily travel with a vehicle provided or reimbursed. The nurse will manage care plans, document activities, and support telemedicine visits to ensure quality, compliant care within scope.

Qualifications

• Must be a licensed nurse in good standing with the board.
• Three years of clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate clearly and collaborate with interdisciplinary care teams.

Responsibilities

• Facilitate receiving medical records from patients’ providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management and initiate a Plan of Care.
• Coordinate care and communicate with providers; reconcile meds.
• On-call may be required and travel between homes and facilities.
