Job summary

Frontier Home Health, LLC is seeking a Registered Nurse to provide in-home skilled nursing care across Pueblo, El Paso and Fremont counties in Colorado. You will collaborate with an interdisciplinary team to deliver high-quality patient outcomes in accordance with CMS and state regulations.

Two years of RN experience, CPR, and a valid driver’s license are required. A $5,000 bonus is available for full-time candidates, and KanTime EMR experience is preferred to support timely charting and care

Qualifications

• Graduate of an accredited school of nursing.
• Current state RN licensure.
• Two years of RN experience, preferably in home health care.
• Current CPR certification and TB test within past 12 months.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Provide skilled nursing in patient homes according to plan of care.
• Assess patient needs and document findings during visits.
• Communicate changes to Case Manager and physician.
• Ensure accurate documentation and compliance with policies.
• Educate patients and families on pain management and medications.
