Job summary

Frontier Home Health is seeking a Registered Nurse to provide in-home skilled nursing care across Pueblo and surrounding counties. You will work with an interdisciplinary team to ensure continuity of care and positive patient outcomes, following CMS and state regulations.

The role requires an active RN license in Colorado, at least two years of nursing experience (preferably in home health), and strong documentation, communication, and caregiving skills.

Qualifications

• Graduate of an accredited nursing program.
• Current state RN license.
• Two years of RN experience; home health setting preferred.
• Ability to meet CMS Conditions of Participation requirements.

Responsibilities

• Provide skilled nursing in patients' homes per plan of care.
• Assess patient needs and document accurately.
• Communicate changes to Case Manager and physician promptly.
• Educate patients and families on care and medications.
• Ensure compliance with regulatory and agency policies.
