Job summary

LHC Group, a leading post-acute care partner within the Optum family, seeks a Registered Nurse in Home Health to deliver and direct nursing care at patients’ homes under physician orders and agency policy. The role involves coordinating total care with the health team to ensure continuity of patient care across settings.

Responsibilities include plan-of-care development with physicians, supervising aides and LPNs, performing supervisory visits, and completing OASIS assessments.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates and revises the plan of care with the physician and care team.
• Supervises home health care aides and licensed nurses; provides instruction and assigns tasks.
• Performs required supervisory visits and ensures compliance with regulations.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents patient visits per policy and payer requirements; coordinates with payer and care teams.
• Communicates with physicians, patients, and families to ensure quality care and service excellence.
• Participates in on-call and weekend rotations; adheres to utilization management.
