Job summary

Assured Home Health, part of the LHC Group within the UnitedHealth Group family, seeks an RN to deliver and direct nursing care for home health patients. You will coordinate the total care plan with physicians and the health care team while ensuring compliance with laws and agency policies.

You will perform supervisory visits, participate in on-call and weekend rotations, and contribute to high-quality patient outcomes while maintaining comprehensive documentation.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to reliable transportation.
• CPR certification or willingness to obtain within 90 days of hire.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and health care team.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks per regulations.
• Provides supervisory visits and ensures documentation and patient care quality.
