Job summary

HomeChoice Health Services, part of LHC Group, seeks an experienced Registered Nurse to provide and direct in-home nursing care in accordance with physicians’ orders and agency policies.

The RN will coordinate the total plan of care with the health care team, supervise home health aides and nurses, complete OASIS assessments, document visits, and ensure compliance while delivering compassionate, patient-centered care.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification preferred or within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently; strong communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and health care team.
• Supervises care provided by Home Health Aides and licensed nurses; provides instruction and assigns tasks per regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation.
• Documents patient visits per policy and payer requirements; coordinates with the care team.
