Job summary

You will supervise aides, perform OASIS assessments, and work with a care team to ensure continuity of care. A valid RN license and clean driving record are required; the role offers a comprehensive benefits package and competitive compensation.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle.
• Preferred 1+ years of Home Health experience.

Responsibilities

• Develop, implement, and revise the care plan in collaboration with physicians and health care professionals.
• Supervise home health aides and licensed nurses; provide instruction and assign tasks.
• Complete assessments (OASIS), medication reconciliation, and timely documentation.
• Ensure quality of care, coordinate with physicians and family, and participate in on-call rotation.
