Job summary

Lifeline Home Health Care, part of LHC Group, seeks a Registered Nurse for home health to coordinate patient care at home, in collaboration with physicians and the health care team. You will supervise aides, conduct visits, and ensure compliance with policies and regulations.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification, or ability to obtain within 90 days of hire.
• Ability to work independently and maintain good communication.
• Solid written and organizational skills.

Responsibilities

• Develops and revises the plan of care with physician and team.
• Supervises home health aides and LPNs; provides instruction.
• Performs supervisory visits as required.
• Completes comprehensive nurse assessments and medication reconciliation.
• Documents visits per policy and payer requirements.
• Communicates with physicians, patients, and families to ensure quality care.
• Participates in on-call and weekend rotation.
