Idaho Home Health, part of LHC Group and UnitedHealth Group family, seeks an RN to provide in-home nursing care and coordinate overall patient care plans. You will work with physicians and care teams to ensure continuity and quality of care in patients' homes.

Responsibilities include conducting assessments, verifying eligibility, and developing individualized care plans with healthcare providers. A strong emphasis is placed on infection control and patient-centered care in diverse home settings.

Competenze

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, vehicle insurance, and reliable transportation.

Mansioni

• Provide and direct nursing care to home patients as prescribed by physician.
• Coordinate total plan of care with health professionals.
• Perform nursing assessments per program requirements and payer needs.
• Verify eligibility and medical necessity for services per policy.
• Develop and revise individualized care plans with providers.
• Ensure plan of care frequency meets patient needs and obtain physician approval.
