Job summary

Lifeline Home Health Care, part of the Optum family, seeks a Registered Nurse to provide in-home nursing care to patients in their homes, coordinating the total plan of care with physicians and the care team.

You will develop and revise care plans, supervise home health aides, perform OASIS assessments, and ensure accurate documentation. We value compassionate, patient-centered care and professional growth.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Develop and revise the plan of care with the physician and other health care professionals.
• Supervise home health aides and licensed nurses per state and federal regulations; provide instruction and assign tasks.
• Complete OASIS assessments including medication reconciliation.
• Document patient visits per policy and payer requirements; coordinate with the care team.
