Job summary

Island Home Care, part of LHC Group within Optum, is seeking an experienced Registered Nurse for home health in Florida. You will deliver nursing care in patients’ homes, coordinate the overall plan of care with physicians and care teams, and ensure compliance with regulations and policies.

You will supervise home health aides and licensed nurses, conduct required visits, document care, and support patient safety and quality outcomes. A valid RN license and reliable transportation are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid driver’s license, vehicle insurance, and access to dependable transportation.
• CPR certification preferred or able to obtain within 90 days of hire.

Responsibilities

• Provides nursing care to patients in home settings as prescribed by physician and regulatory policies.
• Directs and coordinates the plan of care with healthcare professionals and families.
• Supervises home health aides and licensed staff; conducts required visits and documentation.
• Performs comprehensive assessments (OASIS) and supports accurate coding and charting.
• Maintains infection control standards and ensures quality, safe care delivery.
