Job summary

Island Home Care, part of the LHC Group and within the Optum family, seeks a Registered Nurse for home health in Marathon and the Lower Keys. The RN will manage care plans, collaborate with physicians, and supervise nursing staff to ensure quality patient outcomes.

Required is an unrestricted RN license in the practicing state and a valid driver's license with reliable transportation. The role offers a mix of per-visit and hourly compensation and a comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Coordinate total plan of care with other health care professionals involved in care.
• Complete comprehensive assessments and documentation per policy and payer requirements.
• Supervise home health aides and licensed nurses; provide instruction and assign tasks.
• Participate in on-call and weekend rotation as needed to meet patient needs.
