AdventHealth is seeking an experienced RN for home health case management in Florida. You will assess patients in their homes, develop individualized care plans, and coordinate with physicians to ensure quality outcomes.

Responsibilities include delivering skilled nursing care, coordinating with the care team, and educating patients and families on managing health at home. Travel is required within assigned counties.

Formación

• Associate's degree in Nursing (RN) required.
• Bachelor's degree in Nursing preferred.
• Valid driver's license required.

Responsabilidades

• Provide comprehensive home health nursing care and case management.
• Evaluate patient needs and develop individualized care plans.
• Coordinate with physicians and healthcare team for ongoing care.
