Home Health Registered Nurse – Caring, Coordinated Care

Job summary

Island Home Care, part of LHC Group, seeks a Registered Nurse to deliver and coordinate home health nursing care following physician orders and agency policies. You will develop and revise the plan of care, supervise aides, and ensure high-quality, compliant patient care in the home setting.

You’ll perform assessments (OASIS), document visits, collaborate with the care team, and communicate with patients and families.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.
• 1+ years of Home Health experience (preferred).
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Communicates relevant information timely with appropriate staff and physicians to ensure quality care.
• Participates in on-call and weekend rotation as needed to meet patient needs.
