Job summary

Colorado Plains Medical Center Homecare, part of the LHC Group under Optum family, seeks an RN for home health care. You will provide skilled nursing care in patients' homes and coordinate the total care plan with physicians and the health care team.

Responsibilities include developing and revising plans of care, supervising aides, performing comprehensive assessments, and ensuring documentation accuracy per policy. A valid RN license and reliable transportation are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provides nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies.
• Develops and coordinates plan of care with other health care professionals.
• Performs supervisory visits and guides home health aides and LPNs/LVNs as required by state regulations.
• Completes comprehensive assessments (OASIS) and documents visits per policy and payer requirements.
