Job summary

Optum seeks a Registered Nurse for Colorado Plains Medical Center’s Home Health program to provide and direct nursing care in patients’ homes, coordinating the plan of care with physicians and the health care team. You will supervise aides, perform OASIS assessments, and document visits per policy.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to state and federal regulations
• Provides required supervisory visits and ensures compliance with policies.
• Performs comprehensive assessments (OASIS) including medication reconciliation and documents visits per policy and payer requirements
• Coordinates care with physicians, agency staff, patients, families, and community resources to ensure continuity of care
