Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to reliable transportation.

Responsibilities

• Initiates and revises the plan of care with physicians and health care professionals.
• Supervises home health aides and LPN/LVN staff per regulations.
• Performs supervisory visits and coordinates care with the team.
• Documents visits, assessments (OASIS where applicable), and communicates with patients and families.
• Participates in on-call and weekend rotations and maintains compliance with policies.
