Job summary

BrightStar Care is hiring a Home Health Registered Nurse (RN) to assess patients in their homes, start of care visits, and coordinate multidisciplinary care. The RN will provide wound care, collect specimens, administer medications, and educate patients and families on disease management and safety.

The role requires Texas RN licensure, CPR/BLS, and at least one year of nursing experience. A valid driver’s license and reliable transportation are required for travel between patient homes.

Qualifications

• Current Texas RN license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, organization, and communication skills.

Responsibilities

• Conduct SOC, Recertification, Resumption of Care, Evaluation and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes to physician/c care team.
• Coordinate home health services with physicians, therapists, patients, caregivers, and resources.
• Provide wound care, meds administration, and lab draws as ordered.
• Educate patients and caregivers on disease management, safety, and self-care.
