Travel to patient homes within the territory is required, with a comprehensive benefits package and bonus potential. Earn up to 30,000 in total bonuses over three years (10,000 first year, 5,000 sign-on, up to 6,000 year 2, up to 9,000 year 3; monthly increases after four years).

Qualifications

• Active RN license in state of work.
• 1 year RN experience in home health or similar setting.
• Valid driver’s license; willing to drive ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered home health care in coordination with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
