Job summary

Qualifications
- Active Registered Nurse (RN) license in the state you work.
- One year of RN experience and the clinical expertise to deliver quality patient care.
- Valid driver’s license and ability to spend ~20% of workday driving to patient locations.
Responsibilities
- Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
- Perform patient assessments, medication management, and care planning.
- Educate and empower patients and families on disease management and treatment plans.
- Think critically and ensure thorough EMR documentation.
