Job summary

Nova Leap Home Health - Chickasha is seeking a Registered Nurse to visit independent clients and Veterans in their homes, developing and updating new and ongoing care plans for our caregivers to follow. You will also perform skills assessments for caregivers at hire and annually, contributing to the Quality Assurance Committee.

Qualifications

• Oklahoma RN license required.
• Experience in outpatient home health or hospice preferred.
• Strong knowledge of medical terminology and HIPAA.
• Proven ability in patient assessment and case management.
• Excellent communication and collaboration across multidisciplinary teams.
• HIPAA-compliant documentation standards.
• Ability to travel by personal vehicle to client homes in Lawton, Duncan and surrounding towns.
• Professional, courteous demeanor and love of serving seniors and Veterans.

Responsibilities

• Conduct initial client assessments with vital signs and medical history to determine individualized care plans.
• Conduct care plan reassessments every 6 months or as conditions change.
• Provide education on fall prevention, home safety, dementia and cognitive concerns.
• Participate in quarterly quality improvement committee focusing on client outcomes.
