Job summary

Nova Leap Health Corp. seeks compassionate in-home CNAs/HHAs for part-time or PRN roles serving Chickasha and surrounding Oklahoma areas. You will provide one-on-one care to seniors, individuals with disabilities, and those living with dementia, helping them stay safely at home.

Qualifications

• CNA/HHA certification required.
• Experience in home health, home care, assisted living, nursing homes, group homes, or similar caregiving settings preferred.
• Current CPR and First Aid certifications highly preferred.
• Ability to lift and transfer clients using mobility equipment.
• Reliable transportation, valid driver's license, and current auto insurance.
• Ability to pass background checks.
• Excellent communication and interpersonal skills.
• Dependable, compassionate, patient, and dedicated to high-quality care.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, mobility and transfers.
• Provide medication reminders and assist with medication administration per care plan.
• Prepare nutritious meals per dietary needs and preferences.
• Monitor for changes in condition and promptly report to the office.
• Observe and document vital signs as needed.
• Provide dementia/Alzheimer's care and compassionate companionship.
• Perform light housekeeping tasks and maintain a clean living environment.
• Document services accurately and complete visit documentation.
• Assist with errands, grocery shopping, and transportation to appointments.
