Nova Leap Health Corp. in Oklahoma is seeking a compassionate Registered Nurse (RN) to support our home care and Veterans services in Lawton, Duncan, and surrounding areas.

You will perform in-home assessments, develop care plans, educate families, and ensure HIPAA-compliant documentation while traveling between client homes. Join a dedicated team delivering high-quality, client-centered care in home settings with competitive pay and training.

Competenze

Position Summary

Nova Leap is seeking a compassionate and experienced Registered Nurse (RN) to support our growing home care team. The RN will conduct in-home assessments for independent clients and Veterans, develop and update individualized care plans, perform caregiver skills assessments, and contribute to quality improvement initiatives.

This position is ideal for a nurse who enjoys working independently, building relationships with clients and families, and making a meaningful impact in the community.

Key Responsibilities

• Conduct initial in-home client assessments, including review of health history, vital signs, functional abilities, and safety concerns.
• Develop individualized care plans outlining activities of daily living (ADL) assistance and caregiver instructions.
• Complete client reassessments every six months or sooner if condition changes warrant review.
• Educate clients and family members on fall prevention, home safety, dementia awareness, cognitive concerns, and wellness strategies.
• Monitor client status and communicate recommendations to caregivers, families, and agency leadership.
• Perform caregiver skills assessments upon hire and annually to ensure competency and quality of care.
• Participate in quarterly Quality Assurance and Performance Improvement (QAPI) meetings focused on enhancing client outcomes and service quality.
• Maintain accurate, timely, and HIPAA-compliant documentation.
• Collaborate effectively with clients, families, caregivers, healthcare professionals, and community resources.
• Travel to client homes throughout Lawton, Duncan, and surrounding communities.

Qualifications

Required

• Active and unrestricted Oklahoma Registered Nurse (RN) License
• Current CPR Certification
• Reliable transportation, valid driver's license, and current auto insurance
• Ability to travel throughout assigned service areas
• Strong assessment, critical thinking, and documentation skills
• Professional, compassionate, and service-oriented demeanor
• Passion for serving seniors and Veterans

Preferred

• Home health, hospice, case management, or community health experience
• Outpatient assessment experience in a home health or hospice setting
• Knowledge of medical terminology, physiology, and diagnostic evaluation processes
• Experience developing care plans and conducting patient assessmentsFamiliarity with quality assurance and performance improvement programs
