Hospice Registered Nurse (RN) Case Manager
Position Overview
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager to provide high-quality, patient-centered hospice care across multiple counties in Alabama. The RN Case Manager will oversee patient care plans, coordinate with interdisciplinary teams, provide clinical assessments, and support patients and families throughout the end-of-life care journey.
Schedule & Compensation Details

• Schedule: Monday-Friday, 8:00 AM - 5:00 PM
• Hours: 40 hours per week
• On-Call Requirement: Yes
• Weekend Requirement: Backup call coverage on scheduled weekends
• Productivity Expectation: 25 patient points (PP)
• Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities

• Manage a hospice patient caseload and serve as the primary clinical point of contact.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, social workers, chaplains, hospice aides, and other members of the interdisciplinary team.
• Monitor patient symptoms and provide appropriate clinical interventions.
• Administer medications, treatments, and patient education in accordance with physician orders.
• Ensure timely and accurate clinical documentation.
• Provide emotional support and education to patients and family members regarding disease progression, symptom management, and end-of-life care.
• Participate in on-call rotation and respond to urgent patient needs as required.
• Maintain compliance with agency policies, hospice regulations, and quality standards.
  

Required Qualifications

• Active Alabama Registered Nurse (RN) License
• Current Basic Life Support (BLS) Certification
• Valid Driver's License
• Reliable transportation and ability to travel throughout assigned coverage territory
• Strong clinical assessment, documentation, and care coordination skills
